How to Build a Form
Forms let you collect structured information from applicants at different stages of their journey. You can add forms to portal steps or attach them to the signup page to gather data before an applicant even enters the portal.
Creating a Form
Go to Studio → Forms from the sidebar.

Create a new form to open the form builder.

The builder has three panels:
- Pages panel (left): Manage and reorder your form's pages
- Editor (center): Build out each page with fields
- Fields Library (right): Browse, search, and create fields

Adding Pages
Click the + button in the pages panel to add a new page. Each page has its own title and subtitle that you can edit directly in the center panel. Drag pages in the left panel to reorder them. To delete a page, open its menu and select Delete Page.

Working with Fields
Fields live in the Fields Library on the right. To add a field to a page, drag it from the library and drop it onto the page editor. You can also reorder fields within a page by dragging them.
The library has three filter tabs: All Fields, In Use (currently placed on a page), and Not In Use (available but unassigned). Use the search bar to find fields by name.

Creating a New Field
Click Create New Field at the bottom of the fields library. Each field has two name properties:
- Field Name: An internal label for your team. This is how the field appears in the dashboard and library.
- Field Question: The text displayed to the applicant. Write this as a clear, answerable prompt.

You also configure the Question Field Type (text, single select, multi-select, dropdown, date, boolean, and others), set whether the field is Required, and optionally add a description and placeholder text.

For select-type fields, add at least two options. Drag options to reorder them.
Field Categories
Assign a Field Category to control where the submitted data appears on your sales lead panels in the dashboard. You can create new categories directly from the field editor by clicking Add Category and entering a name.

Removing vs. Deleting a Field
There are two ways to take a field off a page:
- Remove From Page: Unlinks the field from the current page. The field stays in your library and can be placed again later.
- Delete Field (from the library menu): Permanently removes the field from your entire form. This cannot be undone.

Best Practices
- Keep the number of fields per page manageable. Shorter pages lead to higher completion rates.
- Use clear, specific questions for the Field Question so applicants know what to provide.
- Group related fields on the same page and give each page a descriptive title.
Using Forms in the Portal
Forms can appear in two places:
- As a portal step: In the portal editor, add a step with the Form type and select the form you want to use.
- On the signup page: In the signup builder under Form Fields, select a form to display additional fields when applicants first create their account.
